Responsibilities
- Provide crisis intervention and long-term counseling for individuals/families facing mental health challenges
- Develop and implement community outreach programs addressing homelessness and substance abuse
- Collaborate with healthcare providers, schools, and government agencies to create integrated care plans
- Supervise 3-5 junior social workers and conduct regular case reviews
- Maintain accurate documentation and ensure compliance with HIPAA and state regulations
- Lead community workshops on mental health awareness and resource navigation
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- 3+ years clinical experience with vulnerable populations
- LCSW or LSW licensure in Illinois
- Crisis intervention and trauma-informed care certification
- Experience with electronic health records (EHR) systems
- Strong cultural competency in diverse urban environments
- Valid Illinois driver's license with clean record
