Responsibilities
- Provide comprehensive case management for clients facing homelessness, domestic violence, or mental health challenges
- Develop and implement individualized intervention plans using trauma-informed care principles
- Conduct crisis intervention and safety planning for high-risk populations
- Facilitate therapeutic groups focusing on life skills, parenting, and emotional resilience
- Collaborate with healthcare providers, schools, and government agencies to create coordinated support networks
- Advocate for systemic change through policy research and community education initiatives
- Maintain accurate documentation and outcomes reporting for funders and regulatory bodies
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- New York State LMSW license (required) with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed methodologies
- Familiarity with NYC's social service landscape and resource navigation
- Strong cultural competency in serving diverse urban populations
- Excellent documentation skills with proficiency in electronic health records (EHR)
- Valid NYS driver's license and reliable transportation for home visits
