Responsibilities
- Provide comprehensive case management and counseling services to individuals and families facing complex social challenges
- Develop and implement personalized intervention plans using evidence-based practices
- Coordinate community resources and collaborate with healthcare providers, schools, and government agencies
- Conduct crisis intervention and risk assessments for vulnerable populations including youth, seniors, and marginalized communities
- Lead community outreach programs and educational workshops on social wellness topics
- Maintain accurate case documentation and ensure compliance with privacy regulations
- Mentor junior staff and contribute to program evaluation and quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years of direct practice experience in social services
- Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Strong knowledge of community resources and social service systems
- Certification in Trauma-Informed Care and Crisis Intervention preferred
- Experience working with diverse populations including newcomers, LGBTQ+ communities, and individuals with mental health challenges
- Excellent communication skills with ability to navigate complex systems
- Valid Ontario driver's license and access to reliable transportation
