Responsibilities
- Develop and implement comprehensive case management strategies for at-risk individuals and families
- Conduct psychosocial assessments and create tailored intervention plans
- Collaborate with community partners to coordinate housing, healthcare, and employment resources
- Provide crisis intervention and trauma-informed counseling services
- Lead community education workshops on mental health and financial literacy
- Advocate for policy changes at municipal and state levels
- Maintain meticulous documentation and compliance with HIPAA regulations
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with 3+ years post-MSW experience
- Proven success in case management with diverse populations
- Advanced knowledge of Chicago's social service systems and resources
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
- Experience developing and evaluating social service programs
- Exceptional crisis intervention and de-escalation skills
