Responsibilities
- Develop and implement individualized case management plans for clients facing housing instability, mental health challenges, and systemic barriers
- Conduct comprehensive psychosocial assessments and risk evaluations using evidence-based frameworks
- Facilitate crisis intervention and de-escalation for high-risk situations involving domestic violence, substance abuse, or acute mental health episodes
- Coordinate with healthcare providers, legal advocates, and community organizations to ensure seamless service delivery
- Provide trauma-informed counseling and group therapy sessions using modalities like CBT and DBT
- Advocate for policy changes and program improvements through data-driven reporting and stakeholder engagement
- Maintain meticulous case documentation compliant with provincial privacy legislation (PHIPA)
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years of direct practice experience in community social services
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) or equivalent
- Proven expertise in crisis intervention and harm reduction strategies
- Advanced knowledge of Ontario's social service systems including ODSP, VAWA, and mental health resources
- Strong cultural competency with experience serving marginalized populations (LGBTQ+, racialized communities, newcomers)
- Valid Ontario Class G driver's license with reliable vehicle for community visits
