Responsibilities
- Develop and implement individualized case management plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis intervention for diverse populations including veterans, families, and LGBTQ+ youth
- Collaborate with community partners to coordinate resources, housing placements, and healthcare access
- Lead trauma-informed group therapy sessions and workshops on life skills and financial literacy
- Advocate for systemic policy changes through local government engagement and coalition building
- Maintain accurate documentation and ensure compliance with HIPAA and state licensing requirements
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- LCSW (Licensed Clinical Social Worker) credential in Illinois
- Minimum 3 years of direct practice experience with vulnerable populations
- Proven expertise in crisis intervention and trauma-informed care
- Strong knowledge of Chicago's social services ecosystem and resources
- Excellent cross-cultural communication skills and Spanish proficiency preferred
- Demonstrated success in grant writing and program development
