Responsibilities
- Provide trauma-informed counseling to individuals and families facing complex socioeconomic challenges
- Develop and implement personalized care plans with measurable outcomes
- Collaborate with multidisciplinary teams including healthcare providers and legal advocates
- Conduct comprehensive psychosocial assessments and risk evaluations
- Coordinate community resources and connect clients to essential services
- Maintain accurate documentation compliant with HIPAA and state regulations
- Lead crisis intervention protocols for emergency situations
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven success in crisis intervention and case management
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Experience working with LGBTQ+ and immigrant populations
- Advanced knowledge of Chicago's social services ecosystem
- Bilingual proficiency in Spanish preferred
- Strong data analysis skills for outcome measurement
