Responsibilities
- Develop and implement individualized care plans for clients across diverse socioeconomic backgrounds
- Conduct comprehensive psychosocial assessments and risk evaluations
- Facilitate crisis intervention and emergency response protocols
- Coordinate community resources and advocate for systemic policy changes
- Provide trauma-informed counseling and therapeutic support groups
- Maintain accurate documentation and compliance with HIPAA regulations
- Mentor junior staff and lead community education workshops
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and case management
- Familiarity with Chicago's social services landscape and resources
- Strong cultural competency serving LGBTQ+ and immigrant communities
- Advanced knowledge of trauma-informed care methodologies
- Excellent documentation skills and electronic health record proficiency
