Responsibilities
- Develop and implement individualized care plans for at-risk youth and families
- Conduct comprehensive psychosocial assessments and crisis interventions
- Collaborate with cross-functional teams including healthcare providers and educators
- Facilitate therapeutic group sessions and community workshops
- Advocate for clients through legal and social systems navigation
- Maintain detailed case documentation and compliance with HIPAA regulations
- Oversee volunteer programs and mentor junior staff members
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- Licensure as LCSW in Illinois (or ability to obtain within 90 days)
- 3+ years direct experience in child welfare or family services
- Advanced crisis intervention and trauma-informed care certification
- Proven success in grant writing and program development
- Exceptional cultural competency and bilingual Spanish/English preferred
- Strong data analysis skills using case management software (e.g., Cerner)
- Valid Illinois driver's license and reliable transportation
