Responsibilities
- Develop and implement client-centered intervention plans for individuals and families facing complex social challenges
- Conduct comprehensive psychosocial assessments and risk evaluations in high-stakes situations
- Lead multidisciplinary case conferences with healthcare, legal, and community partners
- Provide crisis intervention and trauma-informed support to vulnerable populations
- Design and facilitate community workshops on mental health awareness and resource navigation
- Maintain meticulous documentation while ensuring HIPAA-compliant confidentiality standards
- Advocate for systemic policy changes through research and community partnerships
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Ontario College of Social Workers registration (OCSWSSW)
- Minimum 5 years' direct practice experience in child welfare or mental health
- Crisis intervention certification (e.g., ASIST, QPR)
- Advanced proficiency in electronic health records (EHR) systems
- Bilingual proficiency in English and French (highly desirable)
- Valid Ontario driver's license with reliable transportation for home visits
