Responsibilities
- Provide individual and group counseling to clients facing housing instability, mental health challenges, and economic hardship
- Develop and implement personalized care plans connecting clients with housing, healthcare, and employment resources
- Lead crisis intervention teams for emergency situations including domestic violence and homelessness
- Conduct comprehensive psychosocial assessments and maintain detailed case documentation
- Collaborate with healthcare providers, schools, and government agencies to create coordinated support networks
- Train and mentor junior social workers on trauma-informed care practices
- Analyze community needs to advocate for policy changes and program improvements
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as a Licensed Clinical Social Worker (LCSW) in Illinois
- Minimum 5 years of experience in direct social services with vulnerable populations
- Expertise in crisis intervention, case management, and trauma-informed care
- Strong understanding of Chicago's social services landscape and resource networks
- Experience supervising multidisciplinary teams and program development
- Cultural competency in serving diverse populations including LGBTQ+ and immigrant communities
- Valid driver's license with reliable transportation for home visits
