Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis intervention protocols
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal advocates
- Lead community outreach programs targeting at-risk youth and families
- Maintain accurate case documentation using electronic health record systems
- Supervise 2-3 junior social workers through mentorship and performance coaching
- Analyze community needs data to inform program development and resource allocation
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited institution
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven expertise in crisis intervention and trauma-informed methodologies
- Advanced knowledge of Chicago's social services ecosystem and resources
- Bilingual proficiency (English/Spanish) strongly preferred
- Experience with electronic health records (e.g., Cerner, Epic)
- Demonstrated cultural competency with diverse urban populations
- Valid Illinois driver's license with reliable transportation
