Responsibilities
- Provide intensive case management and counseling for vulnerable populations including youth, families, and refugees
- Develop and implement individualized service plans with measurable outcomes
- Coordinate multi-agency interventions and navigate complex social systems
- Conduct crisis intervention and trauma-informed support sessions
- Advocate for clients within healthcare, legal, and educational institutions
- Lead community outreach programs and workshops on social welfare topics
- Maintain accurate documentation and compliance with state/federal regulations
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW/LSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Strong cultural competency and experience with diverse populations
- Advanced knowledge of Chicago's social service ecosystem and resources
- Exceptional documentation skills and familiarity with electronic health records
- Ability to manage complex caseloads and prioritize competing demands
- Valid driver's license and reliable transportation for home visits
