Responsibilities
- Develop and implement individualized care plans for clients facing homelessness, mental health challenges, and domestic violence situations
- Provide crisis intervention, trauma-informed counseling, and ongoing emotional support to diverse client populations
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and government agencies
- Conduct comprehensive assessments using evidence-based screening tools and documentation systems
- Advocate for clients through resource navigation, benefit enrollment assistance, and housing placement services
- Lead community outreach programs to increase awareness of available social services
- Maintain detailed client records while ensuring strict confidentiality and HIPAA compliance
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensed Clinical Social Worker (LCSW) in Washington State
- Minimum 3 years of experience in direct social services case management
- Expertise in trauma-informed care and crisis intervention protocols
- Strong knowledge of community resources and social service systems
- Experience with electronic health records (EHR) and documentation software
- Cultural competency in working with diverse populations including LGBTQ+ and immigrant communities
- Valid Washington State driver's license and reliable transportation
