Responsibilities
- Develop and implement individualized treatment plans for clients facing complex psychosocial challenges
- Conduct comprehensive psychosocial assessments and crisis interventions
- Coordinate care with healthcare providers, schools, and community organizations
- Provide trauma-informed counseling and case management services
- Advocate for policy changes affecting vulnerable populations
- Maintain accurate client documentation and outcomes reporting
- Lead community workshops on mental health and resource navigation
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW licensure in Illinois (or eligible)
- 3+ years direct practice in child welfare or mental health settings
- Experience with electronic health records (EHR) systems
- Deep understanding of trauma-informed care principles
- Bilingual proficiency in Spanish strongly preferred
- Certification in Crisis Intervention or Trauma-Focused CBT
- Valid Illinois driver's license and reliable transportation
