Responsibilities
- Develop and implement individualized treatment plans for vulnerable populations
- Provide crisis intervention and trauma-informed counseling services
- Collaborate with multidisciplinary teams to coordinate holistic care
- Conduct comprehensive psychosocial assessments and documentation
- Advocate for clients through community resources and policy initiatives
- Lead outreach programs to expand service accessibility
- Maintain compliance with HIPAA and state licensing requirements
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW/LMSW license in New York State
- 3+ years clinical experience with vulnerable populations
- Certification in Trauma-Focused Cognitive Behavioral Therapy
- Proven crisis intervention and case management expertise
- Cultural competency in diverse urban communities
- Strong documentation and reporting abilities
- Valid NY driver's license for home visits
