Responsibilities
- Develop and implement comprehensive case management plans for vulnerable populations
- Provide crisis intervention and trauma-informed counseling services
- Coordinate multidisciplinary care teams including healthcare providers and legal advocates
- Conduct psychosocial assessments and maintain accurate client documentation
- Facilitate support groups and community outreach programs
- Advocate for policy changes affecting underserved communities
- Maintain compliance with HIPAA and state licensing regulations
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW licensure in Illinois
- 3+ years experience in direct practice with diverse populations
- Certification in Trauma-Focused Cognitive Behavioral Therapy
- Proficiency in electronic health records (EHR) systems
- Strong crisis intervention and conflict resolution skills
- Valid driver's license and reliable transportation
