Responsibilities
- Develop and implement personalized intervention plans for at-risk individuals and families
- Provide crisis intervention and mental health counseling using evidence-based modalities
- Coordinate with healthcare providers, schools, and legal systems to create holistic support networks
- Train and mentor junior staff on trauma-informed practices and ethical compliance
- Analyze community data to identify emerging needs and program gaps
- Advocate for policy changes through legislative outreach and community education
- Maintain meticulous case documentation while ensuring HIPAA compliance
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Experience with electronic health records (EHR) systems
- Bilingual proficiency (Spanish/English) strongly preferred
- Valid Illinois driver's license and reliable transportation
- Knowledge of Chicago's social services landscape and resources
- Strong analytical skills for data-driven program development
