Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health crises, and economic hardship
- Conduct comprehensive psychosocial assessments and risk evaluations using evidence-based frameworks
- Facilitate crisis intervention and trauma-informed counseling sessions with diverse populations
- Coordinate community resources including healthcare providers, legal aid, and housing assistance programs
- Advocate for policy changes and systemic improvements through community partnerships and coalitions
- Maintain detailed case documentation and progress tracking in compliance with regulatory standards
- Provide mentorship to junior social workers and contribute to program development initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification required
- Minimum 5 years of experience in direct service social work with vulnerable populations
- Expertise in crisis intervention, trauma-informed care, and motivational interviewing
- Proven ability to navigate complex community systems and secure resources
- Cultural competence in serving diverse populations including LGBTQ+ and immigrant communities
- Strong analytical skills for data-driven case management and program evaluation
- Valid Illinois driver's license and reliable transportation for home visits
