Responsibilities
- Develop and implement comprehensive care plans for clients facing housing instability, substance abuse, and mental health challenges
- Conduct psychosocial assessments and crisis interventions for vulnerable populations
- Collaborate with healthcare providers, legal advocates, and community organizations to create integrated support networks
- Lead trauma-informed therapy sessions for individuals, families, and groups
- Manage caseloads of 25+ clients while maintaining 95% documentation compliance
- Advocate for policy changes affecting social welfare programs
- Mentor junior social workers through monthly case conferences
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in New York State with 3+ years post-licensure experience
- Expertise in trauma-focused CBT and motivational interviewing techniques
- Demonstrated success in housing navigation and benefits advocacy
- Bilingual proficiency in Spanish or Mandarin required
- Experience with electronic health records (Epic preferred)
- Valid NYC driver's license with clean record
- CPR/First Aid certification
