Responsibilities
- Develop and implement individualized care plans for vulnerable populations including at-risk youth, families in crisis, and elderly clients
- Conduct comprehensive psychosocial assessments and risk evaluations
- Coordinate multi-disciplinary interventions with healthcare providers, schools, and government agencies
- Provide crisis intervention and trauma-informed counseling services
- Oversee caseload management while maintaining documentation compliance with state regulations
- Lead community workshops on mental health awareness and resource navigation
- Mentor junior staff and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 3 years of post-licensure experience in direct practice
- Licensure as LMSW in New York State
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Demonstrated crisis intervention and case management expertise
- Proficiency in electronic health records (EHR) systems
- Fluency in Spanish or Mandarin preferred
- Valid driver's license and reliable transportation
