Responsibilities
- Develop and implement individualized treatment plans for clients facing complex social challenges
- Provide crisis intervention and trauma-informed counseling to individuals and families
- Conduct comprehensive psychosocial assessments and coordinate community resources
- Lead case management teams and supervise junior social work staff
- Advocate for policy changes affecting underserved populations
- Maintain meticulous documentation and ensure compliance with regulatory standards
- Facilitate community workshops on mental health awareness and resilience
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- NYS LMSW/LCSW license with minimum 3 years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care models
- Experience supervising multidisciplinary teams and program development
- Strong cultural competency in serving diverse urban populations
- Advanced proficiency in electronic health records (EHR) systems
- Valid NYS driver's license and reliable transportation
