Responsibilities
- Develop and implement individualized care plans for clients facing complex social challenges
- Provide crisis intervention and trauma-informed counseling to at-risk populations
- Coordinate multidisciplinary services with healthcare providers, schools, and government agencies
- Conduct comprehensive psychosocial assessments and maintain accurate client documentation
- Lead community outreach programs to address systemic barriers to social equity
- Mentor junior staff and supervise social work interns
- Analyze program outcomes and recommend evidence-based improvements
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification required
- Minimum 5 years of progressive social services experience
- Expertise in crisis intervention, trauma-informed care, and case management
- Strong cultural competency with diverse urban populations
- Proficiency in electronic health records (EHR) systems
- Valid Illinois driver's license with reliable transportation
- Excellent communication and advocacy skills
