Responsibilities
- Develop and implement individualized treatment plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis interventions
- Collaborate with multidisciplinary teams including healthcare providers, educators, and law enforcement
- Facilitate support groups and community workshops on trauma-informed care
- Advocate for policy changes affecting vulnerable populations at local and state levels
- Maintain accurate case documentation while ensuring HIPAA compliance
- Supervise junior social workers and interns
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW or LSW license in Illinois with minimum 3 years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed practices
- Familiarity with Chicago's social services landscape and community resources
- Experience with electronic health records (EHR) systems
- Bilingual proficiency in Spanish preferred
- Valid Illinois driver's license and reliable transportation
- Strong cultural competency in serving diverse urban populations
