Responsibilities
- Assess client needs and develop personalized intervention plans for individuals and families facing homelessness, mental health crises, and systemic barriers
- Lead community outreach programs, including workshops on life skills, resource navigation, and advocacy
- Provide crisis intervention and case management services with a focus on harm reduction and trauma-informed approaches
- Collaborate with healthcare providers, legal aid services, and government agencies to coordinate comprehensive support
- Maintain accurate documentation and prepare reports for funding bodies and regulatory compliance
- Mentor junior staff and supervise social work interns to develop next-generation practitioners
- Conduct program evaluations and recommend improvements to enhance service delivery outcomes
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 5 years of progressive experience in direct social services with vulnerable populations
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Expertise in crisis intervention, trauma-informed care, and cultural competency practices
- Proven ability to manage complex cases and navigate multi-systemic barriers
- Strong analytical skills for program evaluation and data-driven decision making
- Excellent communication and collaboration abilities with diverse stakeholders
- Valid Ontario driver's license and reliable transportation for community visits
