Responsibilities
- Develop and implement individualized care plans for vulnerable populations including youth, seniors, and marginalized communities
- Conduct comprehensive psychosocial assessments and crisis interventions using evidence-based methodologies
- Collaborate with interdisciplinary teams including healthcare providers, educators, and legal professionals
- Provide trauma-informed counseling and advocacy services to clients navigating complex systems
- Oversee case management activities including resource coordination, documentation, and reporting
- Lead community outreach programs and workshops on mental health, addiction recovery, and social justice
- Maintain strict compliance with privacy legislation (PHIPA, PIPEDA) and ethical standards
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Minimum 5 years of progressive experience in direct client services
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Proven expertise in crisis intervention, trauma-informed care, and cultural competency
- Strong assessment, documentation, and case management skills
- Experience developing and facilitating group therapy sessions
- Valid Ontario driver's license and reliable transportation
- Bilingual (English/French) proficiency considered an asset
