Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Provide crisis intervention and trauma-informed counseling services to at-risk populations
- Collaborate with community partners, healthcare providers, and government agencies to coordinate holistic support services
- Conduct comprehensive psychosocial assessments and maintain accurate client documentation
- Lead outreach programs targeting underserved neighborhoods and marginalized communities
- Mentor junior staff and supervise student interns in clinical practice
- Analyze community needs data to advocate for policy changes and resource allocation
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification in Illinois
- Minimum 5 years of direct practice experience in community-based social services
- Proven expertise in crisis intervention, case management, and trauma-informed care
- Strong cultural competency working with diverse populations including LGBTQ+ communities
- Experience developing and managing grant-funded programs
- Excellent communication skills with ability to navigate complex systems
- Valid driver's license and reliable transportation for home visits
