Responsibilities
- Conduct comprehensive assessments and develop individualized care plans for clients facing homelessness, domestic violence, or mental health challenges
- Provide crisis intervention, counseling, and case management services to diverse populations including youth, families, and veterans
- Collaborate with community partners, healthcare providers, and government agencies to coordinate resources and support systems
- Advocate for client rights and facilitate access to housing, employment, healthcare, and social services
- Lead trauma-informed support groups and educational workshops on life skills and emotional resilience
- Maintain accurate case documentation and comply with HIPAA regulations and agency protocols
- Mentor junior staff and contribute to program evaluation and quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as a Licensed Clinical Social Worker (LCSW) in Illinois
- Minimum 3 years of direct client experience in social services or nonprofit settings
- Expertise in crisis intervention, trauma-informed care, and cultural competency
- Strong assessment, documentation, and case management skills
- Experience collaborating with community stakeholders and navigating social service systems
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Valid driver's license and reliable transportation for home visits
