Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized intervention plans
- Provide crisis counseling, advocacy, and resource navigation for clients facing homelessness, domestic violence, or mental health challenges
- Collaborate with multidisciplinary teams including healthcare providers, educators, and law enforcement
- Facilitate support groups and community workshops on topics like trauma recovery and financial literacy
- Maintain detailed case documentation while ensuring strict confidentiality and HIPAA compliance
- Oversee caseload of 25-30 clients while providing mentorship to junior staff
- Develop partnerships with local organizations to expand service access and referral networks
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Demonstrated success in crisis intervention and trauma-informed care
- Experience with electronic health records (EHR) systems and documentation best practices
- Cultural competency working with LGBTQ+, immigrant, and low-income populations
- Strong knowledge of Chicago's social services landscape and public assistance programs
- Valid Illinois driver's license with reliable transportation for home visits
- CPR/First Aid certification (or willingness to obtain within 30 days)
