Responsibilities
- Develop and implement individualized care plans for vulnerable populations including refugees, low-income families, and at-risk youth
- Conduct comprehensive psychosocial assessments and crisis interventions in high-stakes community settings
- Coordinate multi-agency partnerships with healthcare providers, schools, and government entities
- Provide trauma-informed counseling and advocacy services to clients navigating complex systems
- Lead community education workshops on mental health awareness and resource navigation
- Maintain meticulous case documentation compliant with NYS licensing requirements
- Mentor junior social workers through structured supervision sessions
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- NYS LMSW license with minimum 3 years post-MSW clinical experience
- Proven expertise in trauma-informed care and crisis intervention protocols
- Bilingual proficiency in English/Spanish or English/Mandarin strongly preferred
- Experience with electronic health records (EHR) systems like Cerner or Epic
- Knowledge of NYC social service landscape and public benefits programs
- Valid NYS driver's license and reliable transportation for home visits
- Excellent analytical skills for data-driven program evaluation
