Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized intervention plans for diverse client populations
- Provide crisis intervention, trauma-informed counseling, and ongoing case management to at-risk individuals and families
- Collaborate with healthcare providers, legal professionals, and community organizations to coordinate holistic support services
- Advocate for systemic policy changes and lead community outreach initiatives addressing root causes of inequality
- Maintain meticulous documentation and ensure compliance with HIPAA, state regulations, and agency standards
- Mentor junior staff and contribute to program evaluation and quality improvement initiatives
- Facilitate therapeutic groups and workshops on life skills, mental health awareness, and empowerment
Qualifications
- Master's degree in Social Work (MSW) from an accredited program required
- LCSW/LMSW license in New York State with at least 3 years post-licensure experience
- Proven expertise in crisis intervention, trauma-informed care, and cultural competency
- Demonstrated success in developing and implementing evidence-based interventions
- Exceptional case management skills with ability to navigate complex social systems
- Strong analytical abilities for assessment, documentation, and program evaluation
- Experience in advocacy, policy analysis, and community organizing
- Proficiency in electronic health records (EHR) systems and data-driven reporting
