Responsibilities
- Provide direct client counseling and intervention for trauma, mental health crises, and socioeconomic challenges
- Develop and implement individualized case management plans connecting clients with community resources
- Conduct comprehensive psychosocial assessments and maintain accurate documentation
- Collaborate with multidisciplinary teams including healthcare providers, educators, and law enforcement
- Advocate for clients through systems navigation and policy-level initiatives
- Lead trauma-informed support groups and community workshops
- Monitor and report on program outcomes to ensure quality service delivery
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in the state of Illinois (or ability to obtain within 90 days)
- 3+ years of direct practice experience in crisis intervention and case management
- Proven ability to work with diverse populations including LGBTQ+ communities and refugees
- Strong knowledge of community resources and social service systems in Chicago
- Excellent documentation skills and proficiency with electronic health records
- Valid Illinois driver's license and reliable transportation
