Responsibilities
- Provide intensive case management and crisis intervention for at-risk individuals and families
- Develop and implement personalized care plans addressing mental health, housing stability, and economic empowerment
- Collaborate with community partners, healthcare providers, and government agencies to coordinate resources
- Conduct comprehensive psychosocial assessments and document clinical progress
- Lead trauma-informed support groups and educational workshops for underserved communities
- Maintain meticulous case records while ensuring HIPAA compliance and ethical standards
- Mentor junior staff and contribute to program evaluation and improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in the state of Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care methodologies
- Demonstrated success in case management with vulnerable populations
- Experience developing community partnerships and resource coordination
- Strong assessment, documentation, and multicultural communication skills
- Valid driver's license and reliable transportation for home visits
- Bilingual proficiency in Spanish highly preferred
