Responsibilities
- Develop and implement comprehensive case management plans for at-risk individuals and families
- Provide crisis intervention and trauma-informed counseling services
- Collaborate with community organizations, healthcare providers, and government agencies
- Conduct assessments and advocate for clients across multiple systems
- Supervise junior staff and interns while maintaining clinical excellence
- Manage caseloads of 20+ clients with diverse cultural backgrounds
- Participate in program evaluation and quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Demonstrated expertise in crisis intervention and trauma-informed care
- Experience supervising staff or interns in social service settings
- Strong cultural competency and ability to work with diverse populations
- Proficiency in electronic health records and case management software
- Valid driver's license and reliable transportation for home visits
- Knowledge of Chicago's social service resources and systems
