Responsibilities
- Assess client needs and develop personalized intervention plans using trauma-informed approaches
- Provide crisis counseling and case management for individuals/families facing homelessness, domestic violence, or mental health challenges
- Collaborate with healthcare providers, schools, and government agencies to coordinate services
- Advocate for policy changes and community resources to address systemic barriers
- Conduct outreach programs to connect marginalized populations with essential services
- Maintain detailed client documentation while adhering to HIPAA and agency protocols
- Supervise junior staff and interns to ensure high-quality service delivery
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- 3+ years of direct service experience in social services or related field
- Licensure as LMSW or LCSW in New York State
- Expertise in crisis intervention, trauma-informed care, and cultural competency
- Strong advocacy skills with experience navigating complex systems
- Proficiency in electronic health records (EHR) and MS Office Suite
- Valid driver's license and reliable transportation for home visits
- Bilingual proficiency in Spanish preferred
