Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized intervention plans for vulnerable populations
- Lead crisis intervention and trauma-informed care protocols for individuals and families facing housing instability, domestic violence, or mental health challenges
- Coordinate with healthcare providers, legal services, and community organizations to create integrated support networks
- Design and facilitate evidence-based group workshops on life skills, parenting, and financial literacy
- Advocate for policy changes and systemic improvements through community engagement and data-driven reporting
- Maintain meticulous case documentation while ensuring HIPAA compliance and ethical standards
- Mentor junior staff and supervise practicum students in clinical social work best practices
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification in New York State
- Minimum 5 years of experience in direct practice with diverse populations, including crisis intervention and case management
- Expertise in trauma-informed care, motivational interviewing, and strengths-based approaches
- Proven ability to navigate complex systems (housing, healthcare, legal) to secure client resources
- Strong analytical skills for needs assessment and program development
- Cultural competency in serving marginalized communities (LGBTQ+, immigrants, low-income families)
- Valid New York State driver's license with clean driving record
