Responsibilities
- Develop and implement individualized care plans for vulnerable populations
- Provide crisis intervention and trauma-informed counseling services
- Collaborate with healthcare providers, schools, and government agencies
- Conduct comprehensive psychosocial assessments and documentation
- Lead community outreach programs and educational workshops
- Maintain strict confidentiality and HIPAA compliance

Qualifications
- Master's degree in Social Work (MSW) required
- LCSW or LMSW license in New York State
- 3+ years experience in direct client services
- Expertise in crisis intervention and trauma support
- Cultural competency in diverse urban populations
- Strong case management documentation skills
- Valid driver's license and reliable transportation
- Bilingual (English/Spanish) preferred
