Responsibilities
- Develop and implement individualized case management plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis interventions in high-stakes community settings
- Lead trauma-informed group therapy sessions for diverse populations including youth, veterans, and refugees
- Coordinate multi-agency partnerships to secure housing, employment, and healthcare resources
- Maintain meticulous documentation and compliance with HIPAA and state social service regulations
- Supervise junior social workers and provide mentorship in clinical best practices
- Analyze community needs data to inform program development and policy advocacy
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in Illinois with minimum 3 years post-MSW experience
- Proven expertise in crisis intervention and trauma-informed care methodologies
- Demonstrated success in securing housing vouchers and SNAP benefits for vulnerable populations
- Advanced proficiency in electronic health records (EHR) systems and data analytics
- Experience supervising social work interns and developing training curricula
- Bilingual proficiency in Spanish strongly preferred
- Valid Illinois driver's license and reliable transportation for home visits
