Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Provide crisis intervention and trauma-informed counseling services to diverse populations
- Collaborate with healthcare providers, schools, and government agencies to coordinate holistic support
- Conduct comprehensive psychosocial assessments and maintain accurate client documentation
- Lead community workshops on resource navigation, financial literacy, and wellness
- Advocate for policy changes addressing systemic barriers to social equity
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited institution
- LCSW licensure in Illinois with 3+ years post-MSW experience
- Proven success in case management with vulnerable populations
- Strong knowledge of Chicago's social service ecosystem and resources
- Experience supervising interns and junior staff
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
