Responsibilities
- Provide individual and group counseling to clients facing trauma, addiction, and housing instability
- Develop and implement personalized care plans with measurable outcomes
- Coordinate multidisciplinary services including healthcare providers, legal advocates, and housing agencies
- Conduct comprehensive psychosocial assessments and crisis interventions
- Lead community outreach programs focused on mental health education and prevention
- Maintain accurate electronic health records while ensuring HIPAA compliance
- Mentor junior staff and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW or LSW licensure in Illinois (or eligible within 3 months)
- 3+ years of experience in direct clinical practice with vulnerable populations
- Expertise in trauma-informed care and motivational interviewing techniques
- Proven ability to navigate complex healthcare and social service systems
- Strong cultural competency and experience working with diverse communities
- Excellent documentation skills and proficiency with EHR systems
- Valid Illinois driver's license and reliable transportation
