Responsibilities
- Develop and implement individualized service plans for vulnerable populations including at-risk youth, families, and homeless individuals
- Conduct comprehensive psychosocial assessments and provide evidence-based interventions
- Coordinate community resources and advocate for clients' needs across healthcare, housing, and employment sectors
- Lead crisis response teams during emergencies and natural disasters
- Train and mentor junior social workers and volunteers on trauma-informed care practices
- Maintain accurate documentation and comply with HIPAA and state regulatory requirements
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW licensure in Illinois with 3+ years of post-licensure experience
- Proven expertise in crisis intervention and trauma-informed care
- Strong knowledge of Chicago's social services ecosystem and community resources
- Experience developing grant proposals and managing program budgets
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Bilingual proficiency in Spanish highly desirable
