Responsibilities
- Provide direct counseling and case management to individuals and families facing complex social challenges
- Develop and implement personalized intervention plans addressing trauma, mental health, and socioeconomic barriers
- Coordinate with healthcare providers, schools, and government agencies to ensure holistic support
- Conduct crisis intervention and risk assessments for high-vulnerability populations
- Design and facilitate community workshops on resilience-building and resource access
- Maintain comprehensive documentation while ensuring strict confidentiality and compliance with HIPAA
- Lead outreach initiatives to identify and engage community members in need
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW or LSW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention, trauma-informed care, and family systems
- Strong knowledge of Chicago's social services landscape and community resources
- Experience developing and evaluating evidence-based intervention programs
- Exceptional cultural competency with diverse populations including LGBTQ+ and immigrant communities
- Advanced documentation skills with electronic health records (EHR) systems
- Valid Illinois driver's license and reliable transportation
