Responsibilities
- Develop and implement comprehensive care plans for individuals and families facing housing instability, mental health challenges, and economic hardship
- Provide crisis intervention, trauma-informed counseling, and advocacy services to vulnerable populations
- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal advocates
- Conduct thorough psychosocial assessments and maintain accurate documentation
- Lead community outreach initiatives to identify at-risk populations and connect them with resources
- Mentor junior staff and contribute to program evaluation and improvement
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW or LMSW license in New York State
- Minimum 5 years of direct service experience in social services
- Proven expertise in crisis intervention, case management, and trauma-informed care
- Strong cultural competency working with diverse populations including LGBTQ+ communities
- Excellent documentation skills and proficiency with electronic health records
- Valid New York State driver's license and reliable transportation
