Responsibilities
- Develop and implement individualized care plans for clients experiencing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis interventions in high-pressure community settings
- Coordinate cross-agency partnerships with healthcare providers, legal aid services, and educational institutions
- Lead trauma-informed group therapy sessions for survivors of domestic violence and substance abuse
- Advocate for policy changes through community organizing and legislative engagement
- Maintain meticulous documentation compliant with HIPAA and state regulations
- Mentor junior staff through case consultations and professional development workshops
Qualifications
- Master's degree in Social Work (MSW) from CSWE-accredited program
- LCSW licensure in the state of Illinois
- Minimum 5 years' experience in direct practice with marginalized populations
- Proven expertise in motivational interviewing and cognitive behavioral therapy
- Advanced knowledge of community resources and public assistance programs
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Demonstrated ability to manage complex caseloads with competing deadlines
- Proficiency in electronic health records (Epic preferred) and data analytics tools
