Responsibilities
- Develop and implement individualized case management plans for vulnerable populations
- Coordinate multidisciplinary support services including housing, healthcare, and employment resources
- Conduct comprehensive psychosocial assessments and crisis intervention
- Lead community outreach programs and stakeholder engagement initiatives
- Maintain accurate documentation and comply with HIPAA/FERPA regulations
- Mentor junior staff and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from accredited institution
- LCSW licensure in Illinois (or ability to obtain within 90 days)
- Minimum 3 years direct service experience in community-based social services
- Expertise in trauma-informed care and cultural competency frameworks
- Strong crisis intervention and conflict resolution skills
- Proficiency with electronic health records and case management software
- Valid Illinois driver's license and reliable transportation
