Responsibilities
- Develop and implement comprehensive case management plans for at-risk populations including youth, families, and elderly individuals
- Conduct thorough psychosocial assessments and provide crisis intervention services
- Collaborate with healthcare providers, schools, and government agencies to coordinate client care
- Lead community outreach initiatives and public awareness campaigns
- Maintain accurate documentation and ensure compliance with federal/state regulations
- Mentor junior staff and supervise social work interns
- Analyze community needs data to inform program development and resource allocation
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- LCSW (Licensed Clinical Social Worker) certification required
- Minimum 5 years of experience in direct social services practice
- Proven expertise in trauma-informed care and crisis intervention
- Strong knowledge of Chicago's social service ecosystem and resources
- Excellent communication skills with ability to engage diverse stakeholders
- Experience in program development and outcome evaluation
- Bilingual proficiency in Spanish preferred
