Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct comprehensive psychosocial assessments and crisis interventions for high-risk populations
- Coordinate interdisciplinary services with healthcare providers, legal advocates, and community organizations
- Lead trauma-informed support groups and educational workshops for underserved communities
- Advocate for policy changes and systemic improvements affecting vulnerable populations
- Maintain accurate case documentation and comply with all regulatory requirements
- Supervise junior staff and provide mentorship in evidence-based social work practices
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as LMSW or LCSW in New York State
- Minimum 5 years of direct service experience in urban social work settings
- Expertise in trauma-informed care and crisis intervention protocols
- Proven ability to navigate complex community resources and service systems
- Strong cultural competency with diverse populations including LGBTQ+ communities
- Experience developing and managing grant-funded programs
- Valid NYS driver's license with clean driving record
