Responsibilities
- Develop and implement comprehensive case management plans for at-risk families and individuals
- Conduct psychosocial assessments and provide crisis intervention services
- Coordinate with community partners, schools, and healthcare providers to create support networks
- Advocate for clients' needs through policy engagement and resource navigation
- Lead trauma-informed group therapy sessions for adolescents and adults
- Maintain detailed case documentation and comply with HIPAA regulations
- Mentor junior social workers and supervise practicum students
Qualifications
- Master's degree in Social Work (MSW) from an accredited institution
- Licensure as LCSW in Illinois with 3+ years post-MSW experience
- Proven expertise in crisis intervention and trauma-informed care
- Strong knowledge of Chicago's social services ecosystem and resources
- Experience developing and managing grant-funded programs
- Exceptional cultural competency and bilingual Spanish/English proficiency
- Advanced data analysis skills for outcome measurement and reporting
