Responsibilities
- Develop and implement comprehensive, client-centered care plans to support long-term stability and self-sufficiency.
- Provide clinical oversight and supervision for a team of junior case managers and outreach staff.
- Facilitate cross-agency collaboration with local healthcare providers, housing authorities, and government bodies.
- Monitor program outcomes and prepare detailed impact reports for executive stakeholders and grant funding partners.
- Conduct thorough psychosocial assessments to identify barriers to success and provide immediate crisis intervention.
- Advocate for equitable service delivery while maintaining compliance with all regional social welfare regulations.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related field required.
- Minimum of 4 years of experience in social service case management or non-profit administration.
- Current LICSW or related state clinical licensure preferred.
- Exceptional proficiency in client advocacy, crisis de-escalation, and trauma-informed care practices.
- Proven ability to manage diverse caseloads while meeting strict performance metrics.
- Strong interpersonal skills with the ability to communicate effectively across various socioeconomic backgrounds.
