Responsibilities
- Conduct comprehensive biopsychosocial assessments to identify client needs and risks.
- Develop, implement, and monitor individualized care plans in collaboration with clients and stakeholders.
- Provide crisis intervention, counseling, and emotional support during times of acute distress.
- Coordinate services with external agencies, healthcare providers, and legal resources.
- Advocate for client rights and ensure access to housing, employment, and healthcare services.
- Maintain accurate, confidential, and timely documentation in the case management database.
- Participate in case review meetings and contribute to program improvement initiatives.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited university is strongly preferred.
- Current and valid LCSW or LSW license in the State of California.
- A minimum of 3-5 years of progressive experience in case management or social services.
- Deep understanding of local community resources and social safety net programs.
- Strong conflict resolution, communication, and interpersonal skills.
- Proficiency in using electronic health records (EHR) and case management software (e.g., Salesforce, CareLogic).
